What is PACE? 

The Program of All-Inclusive Care for the Elderly, known as PACE, is a model of health care designed to support older adults who wish to remain in their homes and communities while still receiving the medical and social services they need. PACE brings together all aspects of care under one program, providing a comprehensive and coordinated system that reduces the need for nursing home placement. PACE is both a Medicare provider and an optional Medicaid benefit. For those who enroll, it becomes the sole source of their Medicare and Medicaid services. This structure allows PACE organizations to manage all aspects of a participant’s care, from doctor visits and prescription medications to rehabilitation, transportation, and even nursing home care if necessary. Because the program assumes full responsibility for each participant’s care, services are streamlined, coordinated, and tailored to individual needs. 

At the heart of PACE is its interdisciplinary team model. Each participant is supported by a team of health professionals that may include doctors, nurses, social workers, therapists, and dietitians. This team holistically works together to create and regularly update a personalized plan of care, ensuring that medical treatment, social interaction, and personal support are all addressed. The goal is not only to manage health conditions but also to help participants maintain independence, dignity, and quality of life. Eligibility for PACE is designed to focus on those most in need of the services provided. Individuals must be at least 55 years old, live in the service area of a PACE organization, and meet the state’s requirements for nursing home level of care. At the same time, they must be able to live safely in the community when they enroll. Enrollment is voluntary and can be ended at any time, giving participants and families flexibility in their choices. 

PACE is also unique in its financial structure. Instead of paying for each service separately, the program uses a capitated payment system. This means PACE organizations receive a fixed monthly amount for each participant, allowing them to cover all necessary services without being restricted by traditional Medicare or Medicaid reimbursement rules. This flexibility makes it possible to focus on what participants truly need rather than on what a fee-for-service system would cover. Ultimately, PACE represents more than just a health care program. It is a commitment to treating older adults with respect, compassion, and individualized care. By combining medical, social, and financial resources, PACE enables participants to live at home, stay connected to their communities, and receive the comprehensive support they deserve.
